This SEMTAP Report Template provides a simple and easy-to-follow format for creating your SEMTAP client report. You simply fill in the data you collect from the client, and let the template guide you in where to include the information.

This pediatric intake packet includes sections to gather all necessary information from new clients. If you plan on taking photos or videos of any kind during your therapy sessions, you need to have permission from your client and/or their guardian. This form gives clients an opportunity to express their photo and video preferences for a 3 different purposes, which provides more flexibility for both parents and therapists to capture images and videos from sessions.
